THE SHORT ANSWER

Reconcile supported annual collections and necessary continuing work before selecting relevant physical-therapy clinic comparisons. Billed charges, scheduled visits and gross owner benefit are different measures. The public medical-practice sample reviewed here combines many specialties; it does not establish a physical-therapy multiple. Define payer, receivable, equipment and records scope in the actual proposal.

Distinguish broad market context from clinic comparisons

BizBuySell’s broad medical category includes physical therapists, optometry and many other specialties.

Obtain genuinely relevant closed clinic comparisons with matching location, scale, services, payer/collection definitions, owner role and asset scope. Mark undisclosed fields unknown. A buyer’s strategic interest, commercial-payer percentage, manager or practice age does not independently select a multiple or prove a premium.

Bridge billed charges to actual collectible amounts

Trace completed services through billing, contractual reductions, denials, refunds, settlements and collection on consistent periods. Have billing/accounting specialists review open claims, disputes, recoupments and timing. Do not equate a charge ledger with cash or apply a universal age discount.

Hypothetical arithmetic only: $100,000 − $30,000 = $70,000; less distinct receipts $50,000 leaves $20,000. No other adjustments assumed. Unpaid balance still needs eligibility, dispute, recovery-cost and subsequent-receipt review. Not annual revenue/SDE, collectible-value assurance, an industry recovery rate or client result.
Invented single-cohort receivable illustrationAssumption/result
Assumed original billed charges$100,000
Distinct assumed contractual reductions−$30,000
Stated balance after those reductions$70,000
Distinct receipts already applied−$50,000
Remaining stated unpaid balance$20,000

Reconcile actual full-year payroll, supervision, premises, equipment, billing/provider costs and overhead. Separate cash and accrual definitions and avoid counting a service and its later collection twice. Explain payer mix using actual net realizations and costs rather than a supposedly ideal percentage.

Price necessary clinical and administrative work

Document the owner’s treating, supervision, scheduling, management and referral-contact work. Verify available appropriately credentialed people, recurring compensation/benefits and actual capacity using the coverage guide. Reduced owner hours and seller training are separate from permanent coverage.

Support each adjustment using the earnings bridge. Personal, vehicle, renovation or owner-pay labels alone do not justify removing every cost. Distinguish owner-operator SDE from managed earnings after required replacement work instead of switching the metric at an arbitrary revenue size. Use the strategic offer guide to compare buyer plans and transition obligations.

Compare actual payer, records and offer scope

Review actual buyer/entity, location, clinicians, service and payer conditions with appropriate advisors/authorities. Use the consent register for payer/vendor and premises rights. Inspect offered equipment/software appropriately and assess actual investment needs; a contract label or familiar system does not establish every buyer right.

Have privacy counsel determine permitted evidence, access/custody and migration. Use approved aggregate evidence; an NDA alone is insufficient grounds to upload identifiable files. Define receivables, open claims/overpayment questions, deposits, prepaid visits, refunds and service duties using the delivery guide. Compare cash, conditions and continuing costs rather than promising a certified appraisal or fixed preparation uplift.

Owner questions

What multiple should I use for a physical therapy clinic?

Use supported earnings and genuinely relevant closed clinic comparisons; the broad medical sample is not a PT-specific range.

Are billed charges the same as collectible receivables?

Reconcile actual reductions, receipts, disputes and recovery conditions before assigning a value.

Can I remove all owner pay from managed earnings?

Identify necessary continuing clinical and administrative work and its supported replacement cost.

Put this decision into numbers.

Use explicit assumptions and keep the result as a starting point for your advisors.
